We are an out-of-network provider.  If you have no out-of-network 

benefits we will try to work with your insurance company to request

an exception. Please ask us about this service.

To simplify the reimbursement process, we will check your insurance

and submit statements electronically. Before we check your insurance,

you will be asked to provide us with your signed authorization.
